That quiet is part of the problem.


Recent research estimates that about 9,000 people in the U.S. die each year from heat-related causes, a number far higher than many official counts have suggested in the past. Heat often kills indirectly. It worsens heart disease, strains the lungs, disrupts medications, and can trigger confusion or crisis in people already living with mental health conditions. A death certificate may list cardiac arrest, kidney failure, or respiratory disease, while heat was the force that pushed the body past its limit.


That is why heat is often called a silent killer. It is familiar, easy to underestimate, and increasingly dangerous.

Who faces the greatest risk during extreme heat

Older adults

How heat makes existing health problems worse


Heat affects the whole body. The heart pumps more blood toward the skin to release heat. Sweat pulls fluid and salt out of the body. Breathing can become harder, especially when heat mixes with ozone, wildfire smoke, or pollution. Sleep suffers. Medications may work differently when a person is dehydrated.


That chain reaction can turn a manageable condition into an emergency.


Heat and respiratory disease


Hot air can irritate the lungs. High temperatures can also raise ground-level ozone, a pollutant that can worsen breathing for people with asthma, chronic bronchitis, emphysema, or COPD.


When heat coincides with wildfire smoke or poor air quality, the risk grows. People may cough more, wheeze, feel chest tightness, or need rescue medication more often. Some may avoid opening windows because of smoke or pollution, trapping heat indoors.


Practical steps can help:


  • Check local air quality along with the heat index.

  • Stay inside with filtered or cooled air when possible.

  • Use prescribed inhalers and medications as directed.

  • Call a health care professional if symptoms increase.

  • Seek urgent care for severe shortness of breath, blue lips, chest pain, or confusion.


Heat and cardiovascular disease


The heart carries much of the burden during heat. To cool the body, blood vessels near the skin widen, and the heart works harder. Dehydration thickens the blood and can lower blood pressure. For people with heart disease, high blood pressure, heart failure, or a history of stroke, that extra strain can be serious.


Warning signs may include chest pain, unusual shortness of breath, fainting, palpitations, or sudden weakness. These symptoms deserve immediate medical attention.


Some heart and blood pressure medications can also affect fluid balance. That does not mean people should stop taking them during heat. It means they should talk with a clinician before heat season, especially if they have had dizziness, dehydration, or kidney problems in the past.


Heat and mental health


Heat can affect mood, memory, sleep, and judgment. During long hot periods, people may become more irritable, anxious, exhausted, or confused. Poor sleep can make mental health symptoms worse. People living with dementia or serious mental illness may not recognize that they are overheating or may not take steps to cool down.


Some medications used for depression, psychosis, anxiety, Parkinson’s disease, allergies, and other conditions can interfere with sweating, thirst, or body temperature control. Alcohol and some substances can also increase dehydration and reduce awareness.


Support matters here. A reminder to drink water is useful. So is helping someone get to a cooling center, checking whether the air conditioner works, or making sure they know where to go if their home becomes unsafe.

Drink before thirst takes over


Hydration helps the body cool itself. Drink water regularly during hot weather, especially before outdoor activity. People who sweat heavily may need electrolytes, particularly during long periods of work or exercise.


A few simple habits help:


  • Keep water nearby, not just in the fridge.

  • Drink a glass of water before going outside.

  • Avoid heavy alcohol use during heat waves.

  • Ask a clinician about fluid needs if you have heart failure, kidney disease, or fluid restrictions.


Thirst is useful, but it is not perfect. Older adults and young children may not feel or express thirst clearly.


Stay indoors during the hottest hours


The hottest part of the day often falls from late morning through early evening, though this varies by location. When possible, move errands, yardwork, exercise, and travel to cooler hours.


If a home is not cool, find another indoor place:


  • Public libraries

  • Community centers

  • Shopping centers

  • Cooling centers

  • Houses of worship

  • A friend’s or relative’s home

  • Public buildings listed by local emergency management


Fans can help when the air is warm, but they may not be enough when indoor temperatures are very high. At extreme temperatures, blowing hot air across the body can add stress, especially for older adults or people with health conditions. Cooling the body with a shower, damp cloth, air conditioning, or a cooler public space is safer.


Dress and plan for the heat


Small choices reduce heat stress:


  • Wear lightweight, loose-fitting clothing.

  • Choose light colors when outside in direct sun.

  • Use a wide-brimmed hat and sunscreen.

  • Take frequent breaks in shade or air conditioning.

  • Eat lighter meals if heavy meals make you feel overheated.

  • Never leave children, pets, or vulnerable adults in parked cars.


For outdoor workers and athletes, rest breaks are not a luxury. They are part of basic safety. Heat illness can build over hours, and pushing through symptoms can be dangerous.


Recognize heat illness early


Heat illness often starts with signs that seem easy to dismiss. Act early.


Heat exhaustion may cause:


  • Heavy sweating

  • Weakness or fatigue

  • Cool, pale, or clammy skin

  • Headache

  • Dizziness

  • Nausea or vomiting

  • Muscle cramps

  • Fast pulse


Move the person to a cooler place. Loosen clothing. Offer sips of water if they are awake and able to drink. Use cool cloths, a cool shower, or air conditioning. Get medical help if symptoms worsen or do not improve.


Heat stroke is a medical emergency. Call 911.


Possible signs include:


  • Confusion, agitation, or slurred speech

  • Fainting or loss of consciousness

  • Seizures

  • Very high body temperature

Communities can reduce deaths by planning before heat waves arrive.


Local governments, health departments, schools, nonprofits, faith groups, and neighbors can help by:


  • Opening and publicizing cooling centers

  • Extending library or community center hours during heat emergencies

  • Offering transportation to cool spaces

  • Checking on older adults and people living alone

  • Providing water at public events and transit stops

  • Sharing heat alerts in multiple languages and accessible formats

  • Helping residents apply for utility assistance

  • Creating shaded public spaces with trees, shelters, and water access


Many areas offer programs such as cooling centers, emergency utility assistance, 211 referral lines, local health department alerts, and wellness checks through community organizations. Availability varies by location, so it helps to learn what exists before the next heat wave.


A simple neighborhood plan can be powerful. Know who lives alone. Know who does not have air conditioning. Know who may need a ride. Exchange phone numbers with trusted neighbors. During extreme heat, a two-minute call or knock at the door can reveal a dangerous situation early.

A safer heat plan starts before the forecast turns dangerous


Heat will remain a serious health threat across the U.S., and many communities are already facing longer, hotter summers. But heat deaths are not inevitable. Most heat-related harm can be reduced when people take warnings seriously, prepare early, and look out for one another.


A basic heat plan should answer five questions:


  1. Where can you go if your home gets too hot?

  2. Who will you check on during a heat wave?

  3. Who will check on you?

  4. Do you have water, medications, and a way to receive alerts?

  5. Do you know the signs of heat exhaustion and heat stroke?


The danger of heat is that it can look ordinary until it is not. A hot bedroom, a missed drink of water, a broken air conditioner, or one more hour outside can become the moment the body can no longer cope.


Take heat alerts seriously. Stay cool when you can. Drink water before you feel desperate for it. Watch children closely. Check on older neighbors and relatives. If someone seems confused, faint, or severely ill in the heat, treat it as an emergency.


The silent killer becomes less silent when communities talk about it, plan for it, and refuse to leave vulnerable people to face dangerous heat alone.
